Align default 2FA Login Page more with WordPress and add Templating#35

Right now, the 2FA page doesn’t flow well with WordPress. It’d be great if the default look/feel for the 2FA page could mimic the WordPress login page. iThemes Security Pro does this as you can see here: https://screencast.com/t/m45itg1Eqm1m

In addition (or instead), it would be great if we could create our own template or hook into the template to modify the look/feel of the 2FA page.
